Job Objective/ Summary: Responsible for the care of residents living in assigned Health Services care areas.

We currently have two  positions open for our NOC shift  10:30pm – 6:30am.. 1 Full Time Regular 32+hours per week and the other Part Time Regular 31 – 20 hours per week.

Essential Functions:
Collaborates with Health & Wellness Consultant (RN) in completing initial, quarterly, and change-of-condition resident assessments as required; and updates service plans as appropriate.
Monitors health related needs of residents; collaborates with Health & Wellness Consultant (RN) and interdisciplinary team to optimize resident’s health and modify service plan taking the resident’s and the resident’s family’s wishes into consideration.
Provides supervision, training, coaching, and guidance to direct care staff. Collaborates with Health Services Leadership Team and assists in planning and delivering staff in-services and trainings.
Collaborates with Health & Wellness Consultant (RN) and med passers to ensure medication orders received from resident’s physician are input and followed appropriately; clarifies orders with provider as necessary. Ensures medication and treatment orders go through standard triple check process for safety and accuracy.
Ensures appropriate documentation of all provided nursing services [excluding services provided by the Health & Wellness Consultant (RN)].
Ensures initiation of appropriate preventative and rehabilitative nursing procedures.
Collaborates with the Health Services Leadership Team, providing information regarding resident service issues and problems.
Ensures compliance with relevant state statutes, regulations and licensing requirements for assigned care areas as they pertain to medical and direct care staffing functions.
Interacts with families, pharmacists, physician office staff, home health and others in order to ensure that a resident’s needs are met; promotes good facility public relations during interactions.
Initiates the investigation of resident incidents; identifies safety hazards, and institutes corrective action in a timely manner.
Provides ADL assistance to residents as needed.
Adheres to Friendsview’s policies and procedures; including those covering privacy and security in compliance with HIPAA regulations, and codes of conduct. . Maintains confidentiality and respects residents privacy at all times in accordance with HIPAA regulations.

Job Objective/ Summary: Oversees the day-to-day environment of the Health Care Center to assure quality care is given and resident needs are being met in a way that fosters independence and dignity, protects residents, and is in accordance with the Friendsview’s philosophy of person-centered care. Supervises, provides leadership and promotes teamwork of health care personnel. Administers medications and performs treatments according to physician orders.

Essential Functions:

  • Follows Charge Nurse Routine guidelines for shift to ensure consistency in shift routine. Receives report from previous shift and gives report to the next shift to facilitate continuity of care.
  • Supervises health center staff and directs resident care given by caregivers to ensure quality care is given in accordance with established nursing standards, and facility policies and procedures. Ensures that call lights are answered promptly.
  • Works closely with nursing assistants. Observes skill levels and teaches concepts of nursing care on an informal basis. Assists with nursing assistant performance reviews.
  • Ensures nursing assistants have appropriate information to provide care to residents.
  • Administers, documents and oversees medication administration as ordered, and observes resident reactions. Completes drug and behavior monitoring as per program guidelines.
  • Admits new residents. Performs and documents resident assessments on admission and as needed thereafter.
  • Initiates and updates the resident plan of care.
  • Evaluates residents on the Open Event list, provides care as needed and documents findings.
  • Monitors, documents and reports changes in resident status to physicians and families.
  • Receives, documents and follows through on physician orders.
  • Assists physicians as necessary.
  • Supports the spiritual needs of residents and the work of the Spiritual Care Team.
  • Communicates with and coordinates resident care with outside personnel that are providing services.
  • Responds to outpatient needs as appropriate.
  • Follows up with appropriate staff, residents, or other individuals regarding reported complaints, problems, and concerns.
  • Maintains a safe and comfortable environment for residents, staff and visitors.
  • Performs psychomotor nursing skills, which may include taking vital signs, infection control, administering injections, and assisting with daily living requirements.
  • Follows and enforces back safety precautions for lifting and moving residents and other items.
  • Adheres to Friendsview’s policies and procedures, including those covering privacy and security in compliance with HIPAA regulations, and codes of conduct. Maintains confidentiality and respects residents privacy at all times in accordance with HIPAA regulations.

Other Duties & Responsibilities

  • Knows, understands and carries out infection control and standard /transmission-based precautions including appropriate use of personal protective equipment.
  • Knows, understands and responds to emergencies as per responsibilities identified in the Friendsview Emergency Action Plan.
  • Assists as needed in emergency situations. In case of catastrophic event (fire, flood, earthquake, or other emergency) makes all reasonable efforts to report to campus and provide support as directed by emergency coordinator.

  • Attends and participates in orientation, training, in-service, and educational activities to fulfill CEs and as required.
  • Dresses appropriately, following appropriate dress and uniform policies and guidelines.
  • Must be able to work weekends and holidays as required; and maintain consistent attendance as per job requirements and Friendsview’s policies.

Qualifications and Job Requirements:

  • Job Education, Licenses/Certifications, Knowledge & Experience: Current license to practice nursing in the state of Oregon. Prefer a minimum of one (1) year experience in long term care.
  • Supervising (direct reports and level of supervision): Supervision includes immediate supervision of nursing assistants. Mentors and teaches concepts of nursing care to nursing assistants on an informal basis. Assists with nursing assistants’ performance evaluations. Mentors nursing and CNA students.
  • Decision Making: Decision-making, problem solving, and/or recommendations within scope of position responsibilities. Requires interaction with others to solve problems. Must be able to think, act, and intervene independently in both routine and emergency situations.
  • Interpersonal Skills/Interactions: Communication and interpersonal skills are important, and must be well refined. At times, interactions with others are complex, and may be demanding or difficult. Collaboration and cooperation is necessary.

 

Additional Skills & Abilities:

  • Must be able to demonstrate the knowledge, understanding and skills (including psychomotor nursing skills) necessary to provide nursing care appropriate to the needs of older adults.
  • Must be able to apply the nursing process to problem solving.
  • Must be able to prioritize appropriately and make sound nursing judgments.
  • Must have well-developed written and verbal communication skills in order to interpret written instructions, communicate and document effectively.
  • Must be able to work cooperatively and maintain positive relationships with residents, visitors and staff in all depts.
  • Offers emotional support to residents and their families. Must be able to use tactful, diplomatic communication in potentially sensitive or emotionally charged situations.

Requirements:

  • Must successfully pass the Department of Human Services (DHS) criminal background check and, if applicable, fingerprinting.
  • Must be able to pass a pre-employment drug screen.
  • Must be able to pass the pre-employment reference checks.
  • Must be able to provide the official grade transcripts of the education level provided on the employment application.
  • Must be able to provide a copy of his/her own social security card for W-4 documentation.
  • Must be able to provide applicable documentation(s) for I-9 in order to establish identity and authorization to work in the US.
  • Must have valid driver’s license, personal automobile liability insurance, and acceptable driving record (if required to drive).
  • Must have current CPR certification.

 

Bloodborne Pathogens Exposure Category I: Your tasks routinely involve exposure to blood, body fluids, tissues, or other contaminated materials. In compliance with OSHA standards on blood borne pathogens, Friendsview Retirement community will offer Hepatitis B vaccine to all employees who are determined to be at risk of exposure to blood or other body fluids and follow up to all employees who have had an exposure incident. This vaccine will be provided at no cost to the employee.

 

Physical Requirements and Working Conditions:

    • Physical strength, endurance and motor coordination is necessary for frequent periods of walking, standing, performing psychomotor nursing skills and assisting with lifting, transferring and repositioning residents.
    • Must be able to lift, carry, stand, bend, squat, crawl, reach, and kneel using good body mechanics for sustained periods of time.
    • Must have good vision, hearing, speech and language abilities in order to perform and communicate nursing activities.
    • Frequently lifts up to 35 pounds. Any lifting of 35 pounds or more requires the use of an assistive device and/or physical assistance.
    • Able to use gait belt and lift as appropriate and assist in lifting/pushing residents including 2-person transfers
    • Exposure to unpleasant sights, smells, infectious diseases, and combative residents is possible.
